NCCT OF BRAIN (PLAIN)

CT scan of brain is performed without contrast in thin axial sections.

Findings

POSTERIOR FOSSA: Small focal CSF dense space is seen on right cerebellar hemisphere.

Bilateral cerebellar folias are also mildly prominent.

Rest of the cerebellum and brain stem are normal in attenuation pattern. Basal and CP angle cisterns are normal. Fourth ventricle is central and is normal in shape. Both the I.A.Ms are normal and symmetrical.

SELLA: Sellar structures are normal. No evidence of abnormal SOL or calcification is seen. Clinoid processes are normal. Cavernous sinuses are normal in size.

SUPRATENTORIAL: Patchy ill-defined faint hypodensity noted on left periventricular occipitoparietal region.

Bilateral sulcal spaces, sylvian fissure & basal cisterns are prominent.

Rest of the both the cerebral hemispheres are normal in attenuation and gray and white interface is well maintained. Ventricular system is prominent.

BONE AND SCALP: Bony calvarium is normal. No evidence of fracture or SOL is seen. Overlying scalp is normal.

Impressions

Patchy ill-defined faint hypodensity in the left periventricular occipitoparietal region — likely acute ischemic insult.

Urgent MRI brain with DWI/ADC sequences is recommended for confirmation and assessment of infarct acuity.

Small CSF attenuation focus in the right cerebellar hemisphere, likely representing chronic lacunar infarct / encephalomalacic gliotic change.

Mild diffuse cerebral and cerebellar atrophic changes with prominence of ventricles, cortical sulci, sylvian fissures, and basal cisterns.

Recommendation

Urgent clinical and neurological correlation advised.

MRI brain with diffusion-weighted imaging (DWI/ADC) is recommended for further evaluation of possible acute ischemic change.

Correlation with blood pressure and vascular risk factors advised.
